Seed Disability Services is in partnership with YouCentric Care Group, a leading mobile Occupational Therapy provider across NSW.
Through this partnership, we are able to offer:
- Functional capacity assessments
- Assistive Technology trials & reports
- Home modification assessments & recommendations
- Ongoing therapy (in-home, community & Telehealth)
- NDIS, Medicare & Aged Care OT referrals
- Faster access + reduced wait times
Together, we streamline assessments, communication and reporting—creating a simple, coordinated process for participants, families, and support coordinators.
Trilogy Care – Aged Care Partner
Seed Disability Services also partners with Trilogy Care, a trusted Home Care Package (HCP) provider.
This partnership allows us to:
- Support clients with Level 1–4 Home Care Packages
- Deliver funded aged care services with no out-of-pocket costs (if within package budget)
- Assist private clients and self-funded seniors
- Provide clinical, domestic, personal, transport and social supports under Trilogy’s HCP management
- Offer free onboarding support for new HCP clients wanting to switch providers or set up a new package
Through Trilogy Care, we can organise funded nursing care, personal care, domestic help, social supports, transport, and more, all delivered by Seed Disability Services.
