We provide a full range of NDIS and Aged Care approved supports, tailored to your goals. Services include:

All staff are carefully screened and trained. They must hold:

  • First Aid and CPR certifications
  • Police Check and NDIS Worker Screening Check
  • (Where required) Working With Children Check

We also provide ongoing training to ensure our team is always up to date with best practices in care and support.

Applying is simple. Contact My Aged Care on 1800 200 422 to create a profile and book your assessment. Once approved, you can nominate Seed Disability Services as your provider, and we’ll help you every step of the way to set up your personalised plan.

We make sure your package is tailored to your specific needs.

Your support level is determined during your My Aged Care assessment. There are four levels of care, ranging from basic to high-level needs. Once approved, we’ll work with you to make sure your services match your goals and lifestyle.

Yes. We’ll support you through the entire NDIS journey — from understanding eligibility, gathering documents, and completing the Access Request Form, to developing your care plan once you’re approved.

Yes. Our Support Coordinators help you understand your NDIS plan, connect with providers, and make sure your supports are delivered effectively. We’ll work with you to build skills, resolve issues, and get the most from your plan.

Care plans are reviewed at least once a year, but reviews can happen sooner if your needs change. Seed will always check in to make sure your plan continues to meet your goals.

Yes. Our team is trained and experienced in supporting clients with a wide range of disabilities and health needs. We also provide ongoing training to ensure staff can deliver safe, respectful, and effective care.

FAQs – Funding for Aged and Disability Care
Aged Care & NDIS General

Through My Aged Care, you can access funding via Home Care Packages or the Commonwealth Home Support Programme. Both provide financial support based on your care needs and are means-tested to determine your contributions.

The NDIS offers funding tailored to your disability needs, covering Core Supports for daily activities, Capacity Building for skill development, and Capital Supports for assistive technology. Eligibility depends on meeting specific criteria and completing an assessment.

For My Aged Care, contributions are based on your income and assets. The NDIS generally covers necessary support costs, though some services (like transport) may require small contributions.

Home Care Packages

Contact My Aged Care on 1800 200 422 to create a profile and arrange an assessment. Once approved, you can choose Seed as your provider, and we’ll help you develop a personalised care plan to start services.

NDIS Plan Management

Inform your NDIA Planner or Local Area Coordinator (LAC) during your planning meeting. If approved, your NDIS plan will include funding for plan management services, allowing you to select a plan manager to help manage your NDIS funds effectively.

A plan manager handles the financial aspects of your NDIS plan, including:

  • Processing invoices
  • Making payments to service providers
  • Providing regular financial statements
  • Helping you budget and spend in line with your plan

They can also provide financial guidance and assist in understanding your spending.

Service Quality & Support

No. Services are available for people of all ages, including seniors, people with disabilities, and those needing temporary or ongoing assistance.

Yes. You’re welcome to visit. We recommend booking an appointment to ensure a care coordinator is available to meet you.

Yes. Private Care can be scheduled to suit your preferences, offering flexible options.

Costs vary depending on the level of support and duration of services. Contact us for a detailed quote.

Yes. You can receive Private Care services while your government funding is being processed.

Once approved, government funding will commence, and your care plan will be adjusted accordingly.

To include Support Coordination in your NDIS plan, you need to request it during your planning meeting with your NDIA Planner or Local Area Coordinator (LAC). They will discuss your individual needs and determine if Support Coordination is suitable for you. If approved, your plan will include funding for this service, and you can select a Support Coordinator to assist you.

Staff Training & Quality

Yes. Our care workers receive training tailored to the needs of the individuals they support. Their skills and qualifications vary by role, and ongoing professional development ensures they continue to deliver safe, high-quality care.

iCare Support

iCare offers support for individuals affected by workplace injuries, motor accidents, or dust diseases in NSW. Services include:

  • Workers’ compensation
  • Motor accident injury treatment
  • Dust disease care
  • Medical treatment, rehabilitation, financial compensation, and long-term care

Applications can be made directly via the iCare NSW website, where guides and forms are available. For additional help, you can contact iCare or reach out to us for guidance.

Eligibility depends on the program. Typically, individuals injured at work, in a motor vehicle accident, or through exposure to dust diseases in NSW may qualify. Each program has specific criteria outlined on the iCare NSW website.
