We provide tailored supports to help participants achieve their goals and live independently, including:
NDIS Referral
For participants need assessments, therapy, AT, home modifications etc.
Private Care / Medicare
For GP referrals (EPC/CDM), DVA clients, private health insurance, or self-funded clients.
Aged Care
For Home Care Package (HCP) or CHSP funded clients. Falls prevention, home safety.
